Angiopulmonography is an X-ray contrast study of pulmonary vessels aimed at determining the vascular pattern within the pulmonary artery system and assessing vascular blood flow.

Traditionally considered the optimal method for establishing indications for endarterectomy, it is particularly useful in patients with suspected chronic pulmonary embolism that has not been confirmed by lung scintigraphy. In such cases, performing angiopulmonography is advisable to diagnose distal obstructions in the pulmonary vascular bed.

The procedure is uncomplicated when conducted by highly specialized personnel, utilizing modern contrasts and allowing for selective contrasting of the right and left main branches of the pulmonary artery with analysis in multiple projections. It is carried out in a specially equipped X-ray operating room, taking precautions, as the introduction of a contrast agent has the potential to provoke a pulmonary hypertensive crisis.
